_*What You Will Own…*_
Providing and directing nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and policies.
The LPN coordinates the total plan of care with other clinical professionals and helps achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources.
As the primary point of contact for our clients, you are personable and understand their sense of urgency and how to balance their demands with the existing daily deliveries.
Qualifications:
* Current license/registration by the state of hire as a Licensed Practical Nurse
* Preferred one year experience in Nursing
* Prior Home Health Experience also preferred but not required
* Ability to complete Competency and Proficiency Skills Evaluation
* CPR Certification
* Excellent verbal and written communication
* Valid drivers license with a clean driving record and a clean, functioning vehicle
* Current TB test
* Valid Driver’s license and reliable automobile
* Proof of current automobile insurance (MVR checked)
* Criminal background check clearance
* Must be vaccinated for COVID-19 or be eligible for an exemption on the basis of sincerely held religious belief or medical reason.
