Ambulatory Clinic-Patient Care Coordinator-(FT)
Provides care for all patients by developing, monitoring and evaluating care specific to the age of the patient population served in the medical clinic.

S/he communicates and works directly with all providers and any staff members that are directly involved with the patient’s care, in regards to any changes in the patient's clinical condition and care plan.
S/he participates as an active team member in the quality programs that physician clinics are involved in.
Medicare Accountable Care Organization, Medicaid Value Care Organization and actively ensures that the NCQA Patient Centered Medical Home accreditation is maintained at the clinic’s where it is a requirement.
S/he is able to perform general nursing duties and participates in performance improvement activities.
S/he ensures that quality care is provided in an efficient and safe manner, consistent with standards of care.
S/he demonstrate performance consistent with the mission, philosophy and goals of the Madison Memorial Hospital and remains flexible to changing systems.
S/he is expected to demonstrate quality and effectiveness in work habits and clinical practice and treats staff, physicians, patients and families with consideration and respect.
Schedule: Full time
Monday-Friday 8am-5pm

Required Skills:
1.
Must demonstrate exemplary care of patients and professional respect for medical providers and co-workers.
2.
Knowledgeable in medical terminology and electronic medical records.
3.
Certification: Must maintain a current certification, in EMT, CNA, CMA, LPN, RN
4.
Basic knowledge of Microsoft Word, Outlook, Excel, and basic Windows based programs
Minimum Work Experience:
1.
A minimum of one year of work experience in a medical office is preferred.
Minimum Education:
1.
High school diploma.
2.
Graduate of an accredited school.
