Idaho Falls Community Hospital is looking for an Access Center Coordinator to join our team! JOB SUMMARY: Serves as an incoming and outgoing portal for physicians needing information, connection to hospital based physicians, transport or transfer of patients, coordinates connecting admitting physician to House Supervisor or the appropriate charge nurse to take medical orders, internal bed assignment for newly admitted patients and other functions to assist the patient/physician access into and out of the hospital.

DUTIES AND RESPONSIBILITIES: Access Center Coordinator 1.
Coordinates all referrals and utilizes established admission criteria to evaluate appropriateness of admission for incoming direct admits.
2.
Working with referring/admitting physician, Nursing House Supervisor and the Nursing Managers, assign new intake to a floor, unit and bed.
3.
Direct incoming calls to Hospitalists, ED Physicians or Intensivists as appropriate.
4.
Receive phone calls from outside medical facilities such as physician offices, other hospitals or urgent cares.
Provide answers when appropriate or facilitate the discussion when necessary with the current in-house personnel.

) 5.
Verify Emergency Medical and Treatment and Labor Act (EMTALA) compliance with referrals and transfers.
6.
Record and accurately document incoming calls in the call log.
7.
Coordinates connecting admitting physician to House Supervisor or the appropriate charge nurse to take medical orders.
8.
Facilitate all patient transfers inside, into or out of the hospital, both emergent and nonemergent as appropriate.
Coordinate data and patient information; communicate with EMS or transport personnel as well as the nursing house supervisor, unit manager or unit supervisor.
9.
Facilitate peer-to-peer discussions when receiving or sending a patient for transfer from/to a different facility.
10.
Is responsible for EMR patient bed assignment within the hospital while working with Unit Manager, Nursing House Supervisor or Director of Nursing as appropriate.
11.
Maintains appropriate logs as directed for hospital compliance or quality improvement measures.
12.
Provides accurate and timely census reports.
13.
Facilitates and manages on-call list.
Coordinates physicians and hospital medical staff providing services in that capacity.
14.
Coordinates with contracted outside telehealth services as directed by in-house physicians.
15.
Coordinates all non-emergent patient transports with appropriate transport service and receiving or sending facility.
16.
Notifies the on-call Hospitalist of different activated codes they are responsible to respond to.
17.
Facilitates communication with outside law enforcement as needed or requested.
18.
Facilitates enactment of safety or disaster protocol tree when directed.

Education: Must have a current, unencumbered Idaho License in your medical field.
BLS certification required.
Experience: 3-5 years of experience as an Access Center Coordinator, nursing (CNA, LPN, RN), or other acute care medical experience.
Must be proficient in medical terminology.
Equipment/Technology: Ability to use EMR (Cerner) as well as a multi-line phone system and radio for EMS communication.
Language/Communication: Must be able to accurately communicate in a high stress environment.
Must be able to accurately relay information between providers and other nursing staff.
Mathematical Skills: Ability to perform basic math.
Mental Capabilities: Able to facilitate communication and access into and out of the hospital.
Facilitates transports, transfers and incoming patient orders.
Must have good knowledge of appropriate guidelines and regulations related to patient care, EMTALA, HIPAA, Hospital Policies and Protocols.
Interpersonal: Able to communicate in a positive and professional manner with physicians, fellow nurses and other healthcare professionals.
Performance: Meets the standards in reference to professionalism and competency.
